Employee W2
For groups with 1 & 2 enrolling employees, Premera requires that the group provides a W2 from one of the
enrolling employees.
- Must be for one of the enrolling employees
- Cannot be hand written
- Tax ID (EIN), company name and address must match the master application
- Must include dollar amounts in the Social Security and Medicare tax deduction boxes
- W2 must be for the prior year (groups enrolling in 2022 must present a W2 from 2021)
